This project consisting of renovation of a relocated house into a
daycare "Green Mountain Child Care" building with municipal water
service and new on -site sewage disposal system (maximum 30
children), Iocated on Airport Parkway in the City of South
Burlington, Vermont is hereby approved under the requirements of
the regulations named above, subject to the following conditions:
GENERAL
(1) The project must be completed as shown on the plans: by
Harold J. Stewart, P.E., Jennison Engineering Inc., Sheet M-
1, "Mechanical Plan" dated October 10, 1985 revised October
16, 1985, and by Kenneth Pinkham, P.E., Pinkham Engineering
Associates Inc., Sheets 1 and 2, "Disposal Area Design,
Property now or fomerly of George Brady" dated October 1985
and by R.A. Wolback Engineering, Green Mountain Child Care
Sheet E-1 and E-2, "Lighting Layout" dated October 8, 1985
and which have been stamped APPROVED by the Division of
Protection. No alteration of these plans shall be allowed
except where written application has been made to the Agency
of Environmental Conservation and approval obtained.
(2) A copy of the approved plans and the Land Use Permit shall
remain on the project during all phases of construction and,
upon request, shall be made available for inspection by
State or Local personnel.
(3) The Vermont Department of Health is to be contacted in
regard to any regulations and/or licenses required by their
department. (They may be reached at 60 Main Street,
Burlington, Vermont, or by phoning 802-863-7220.)
WATER SUPPLY
(4) The project is approved for connection to a municipal
type water system approved by the Department of Health
provided that all construction is in accordance with the
approved plans and the Environmental Protection Rules,
Effective September 10, 1982. One copy of a record drawing
shall be submitted prior to use to show the location, size,
and type of water line installed.

SEWAGE DISPOSAL
(5) The project is approved for the on -site subsurface disposal of
wastewater within the soil boring and percolation testing
area indicated on the certified plans. No buildings, roads,
water lines, or other construction that might interfere with
the installation or operation of the sewage disposal field
is permitted on or near the continuous area. All isolation
distances as set forth in Environmental Protection Rules,
Chapter 8, Section 8-08, shall be adhered to.
(6) The installation of the sewage disposal system shall be
inspected by a professional engineer registered in the State
of Vermont who shall report in writing prior to use, that
the work has been done in accordance with the approved plans
and the permit.
(7) The sewage disposal system shall be constructed maintaining
a minimum of 25 feet from the pump station to the water
service line; and where the force main crosses the water
line, a minimum of 2 feet of vertical separation must be
maintained, with the force main below the water service line
if possible.
(8) This permit shall in no way relieve you of the obligations
of Title 10, Chapter 48, Subchapter 4, for the protection of
groundwater.
(9) The applicant is reminded that all plumbing material and
workmanship must meet the standards of the Environmental
Protection Rules, Chapter 9, Plumbing; the National Plumbing
Code; and the requirements of the Vermont Fire Prevention
Section of the Department of Labor and Industry.
(10) No alterations to the existing building which would change
or affect the interior waste plumbing, water supply, or
wastewater disposal shall be allowed without prior review
and approval from the Agency of Environmental Conservation.
(11) A small sink should be tied into the 3 inch stack vent on
second floor in the sleep room above the mechanical room.
This will eliminate the 3 inch vent being on a dead end.
The 2 inch back vent from the basement which tees into the 3
inch stack vent shall be moved up to a point 6 inches above
the flood rim of the new sink in the sleep room.

(12) The two lavatories located on the main floor shall be vented
as indicated on the revised plan in accordance with the
plumbing code.
(13) One ventilation vent installed in the mechanical room and
supplying fresh air and exhaust to the two classrooms can be
accepted as a revision.
(14) The bathroom in the basement is not approved for use at this
time. Prior to using this bathroom, complete and
satisfactory plans by a professional engineer registered in
the State of Vermont shall be submitted, reviewed and
approved showing the exterior sewage disposal system
connected to this sewer pipe. The maximum number of children
allowed in this building is 30 until these fixtures or other
fixtures are proposed and approved by the Division of
Protection.
