We have completed our inspection of the new water and wastewater service lines that were installed in April andMayof2014at9HighlandTerrace,South Burlington,VT,and per item 2.1 of WW-4-4141,I am providing writtencertificationofthefollowing:
I hereby certify that in the exercise of my reasonable professional judgment the installation-relatedinformationsubmittedistrueandcorrectandthatthepotablewatersupplyandwastewatersystems wereinstalledinaccordancewith:the permitted design and all permit conditions;or record drawings and suchrecorddrawingsareincompliancewiththeapplicablerules,were filed with the Secretary,and are inaccordancewithallotherpermitconditions;were inspected;were properly tested,and have successfully metthoseperformancetests.
