This project, consisting of adding a parcel of land,
(approximately 60' by approximately 695' by 56.09' by 674.28') -
to a pre-existing lot (124.97' by 674.28' by 124.98' by 675.52')
with a pre-existing residence for a total of approximately 3.2
acres located off Spear Street in the City of South Burlington,
Vermont is hereby approved under the requirements of the
regulations named above, subject to the following conditions.
GENERAL
1. This permit does not relieve the permittee from
obtaining all other approvals and permits as may be required
from the Act 250 District Environmental Commission, the
Department of Labor and Industry (phone 828-2106 or 658-
2199), the Vermont Department of Health (phone 863-7220),
and local officials PRIOR to proceeding with this project.
2. The project shall be completed as shown on the plans Project
93161 "Location Survey -Many Campbell" dated 02/28/94, last
revised 02/28/94 prepared by Krebs & Lansing Consulting
Engineers, Inc. and which have been stamped "approved" by
the Wastewater Management Division. The project shall not
deviate from the approved plans without prior written
approval from the Wastewater Management Division.
3. Each prospective purchaser of each lot shall be shown a copy
of the approved plot plan and this Subdivision Permit prior
to conveyance of the lot.
4. The conditions of this permit shall run with the land and
will be binding upon and enforceable against the permittee
and all assigns and successors i.n interest. The permittee
shall_ be responsible for the recording.of this permit and
the "Notice of Permit Recording" in the City of South
Burlington Land Records within'30 days of issuance of this
permit and prior to the conveyance of any lot subject to the
.jurisdiction of this permit.

5. This project has been reviewed and is approved for the
existing single family residence on the approved lot.
Construction of other type dwellings, including public
buildings, duplexes, and condominium units, is not allowed
without prior review and approval by the Wastewater
Management Division and such approval will not be granted
unless the proposal conforms to the applicable laws and
regulations.
6. This permit is being issued pursuant to Section 2.02.N (2)
of the Environmental Protection Rules. Due to the
negligible potential for adverse environment impact, the
Division is issuing the permit without requiring submission
of detailed information on the water supply and sewage
disposal systems. The Division has relied solely upon the
information submitted in the application and the review has
been limited. The Division maintains continuing
jurisdiction over the project.
SEWAGE DISPOSAL
7. If a replacement wastewater disposal system is required for
the lot subject to this permit, the landowner may install a
system approved by the municipality without further review
or approval by the Division.

09/27/94, including a fee of $50 paid for by Check Number 88192. This
application falls under the Single Lot Subdivision Program Area, and under
the Performance Standards for this program area, we have 30 days of 'in-
house' time for our review of your application.
If we need further information from you in order to reach a decision
on your application, the time we wait for your submittal of that
information does not count against the allowable in-house time specified in
the performance standards.

received a decision on your application within the 30 days in-house, please
