Pursuant to Public Building permit #PE-4-0557, conditions #4, #5 and #6 specifically state certain requirements which mast be set regarding the construction of the moused type wastewater disposal system for the budding. Daring my visit I could not see any evidence that a sound type system has been constructed. Further, Engineer, Lance Llewellyn in his letter of March 5, 1981 refused to certify that the wastewater disposal system ham been constructed according to plan. We understood that a proper system vould be installed during the summer of 1981, however, it has not been dome. At this point, ve are considering the filing of a civil action against you to bring the project into compliance with the coQditions of the permit and other penalties as provided for in the law, 18 V5A 11303, 1305. If your intend to bring your waste water disposal system into compliance with the permit please forward your ;mitten proposal to an before December 11, 1991. Barnier, on site sewer, lot #15, Berard Drive Dear Clem: ZONING ADMINISTRATOR 658.2486 9) After the discussion, regarding the leach bed design with Lance Llewellyn and a follow up inspection of it, I understand the concern by Ernest Christianson. On December 10, 1980, this office conducted an inspection of the pipe work being done by Alan Berard. We discussed the finish work of the system and Alan Berard indicated that it would be done according to the approved plan. It appears that a floor drain was piped to the leach bed, and in fact the pipe is exposed. Footing drains are also piped to the same general area of the leach Lx�d. The amount of water from the two drains will definitely cause problems to the leach bed. As I explained to you, the system must be designed and built to accommodate the building regardle SS of the user. Should a temporary permit be issued for the building by Mr. Christianson, the City would became a party to that permit and will require that a new leach bed be constructed when the area becomes workable. If you have any questions, please don't hesitate to call me. Site plan application Mobil oil Corp. for demolition of existing building and construction of a 1089 sq. ft. Mobil Mart as station with snack shop) with 3 pump islands, 811 Willison Rd. Ms. Pugh moved to table the site plan application of Mobil Oil until 1 December. Ms. Hurd seconded. Motion passed unanimously. 2. Site Plan application of Richard Barnier'for construction of a 840 sq.. ft. addition to existing truck terminal, 15 Berard Drive Scott Barnier explained the addition is needed for storage. Mrs. Lafleur noted $180 in landscaping would be required which would be 1 large tree. She said there are a good number of trees on the site. Ms. Hurd said she would be willing to waive the landscaping. Mr. Barnier confirmed that the piles of debris on the rear of the property have been cleaned up. Circulation and Parking: An eleven space parking lot is shown; 8 are required. The plans show adequate circulation with 54 foot wide aisles. Landscaping: The $6000 addition requires $180 in new land- scaping. The site has some landscaping in the front lawn but could use an additional large tree or two. Building coverage is 7.7% and total coverage is 49.9%. The rear of the property could be cleaned up a bit. Other: See Bill Szymanski's and Chief Goddette's comments. 4) BLODGETT ZONE CHANGE This public hearing has been rescheduled for November 24, 1987 due to an error in the public warning notice. 5) LARKIN, COMMUNITY CARE FACILITY, 20 HARBOR VIEW ROAD John Larkin proposes to construct a 2 story, 39,000 square foot total Community Care facility on a 2.5 a -re parcel (lots #7 and 8) on Harbor View Road. The facility will have 30 community care units and 17 efficiency apartments. The property is zoned Commercial -Two and this is a conditional use that received ZBA approval on October 19, 1987. (ZBA minutes enclosed) Access: Access is shown from a 30 foot wide curb cut on the eastern most end of the property. A sidewalk is shown along the frontage and to the front porch as well as to the interior doors of the building. Just completed curb and sidewalk work along Williston Road should not be disturbed. Plan shows two 5 foot curb radii changed to 15 foot. This change will not discourage left turns and is not worth removal of new paving and resulting in a patch. 2. Plan is a significant improvement over what presently exists on the site. 3) BARNIER PROPERTYNADDITION, BERARD DRIVE 1. No comments. 5) HARBOR VIEW COMMUNITY CARE CENTER, HARBOR VIEW DRIVE 1. The sidewalk along Harbor View Drive must be 5 feet in width and constructed one foot away from the property line. It should also be continuous across the entrance drive. 2. The 6 inch watermain may not be sufficient to produce the required fire flow for the size of structure planned. The pipe size'may have to"be 8 inch. y 3.
