SITE PLAN APPLICATION #SP-13-02
FINDINGS OF FACT AND DECISION
Cupola Golf Course, Inc., hereinafter referred to as the applicant, is seeking site plan
approval to amend a previously approved plan for a two (2) 32 unit multi -family
dwellings and a 40 unit congregate care housing facility. The amendment consists of
revising the phasing plan for the three (3) buildings, 237, 259, 261, & 313 Quarry Hill
Road.
Based on the findings below, the Administrative Officer finds, concludes, and decides the
following:
FINDINGS OF FACT
1. The applicant is seeking site plan approval to amend a previously approved plan
for a two (2) 32 unit multi -family dwellings and a 40 unit congregate care
housing facility. The amendment consists of revising the phasing plan for the
three (3) buildings, 237, 259, 261, & 313 Quarry Hill Road.
2. The owner of record of the subject property is the Cupola Golf Course, Inc.
3. The application was received on January 15, 2013.
4. The plan submitted consists of a two (2) page set of plans, page one (1) entitled,
"Quarry Hill Subdivision QHC Lot#1 Quarry Hill Road, South Burlington, Vermont"
prepared by Ruggiano Engineering, Inc., dated 1/15/13.
5. The revised phasing plan provides for the minimum number of parking spaces
needed for each phase as follows:
a. Phase I — 65 spaces
b. Phase II — 71 spaces
c. Phase III — 73 spaces
6. The landscaping plan does not indicate the three (3) phases. This information is
needed to determine the extent of the landscaping to be planted with each phase.
The landscaping plan should therefore be revised to include the phasing plan and
also be revised to divide the planting schedule into the three (3) phases including
the budget for each phase.

DECISION
Based on the above Findings of Fact, the Administrative Officer hereby approves site
plan application #SP-13-02 of Cupola Golf Course, Inc., subject to the following
conditions:
1) All previous approvals and stipulations which are not superseded by this approval
shall remain in effect.
2) This project shall be completed as shown on the plan and shall be on file in the
South Burlington Department of Planning and Zoning.
3) The plans shall be revised to show the changes below and shall require approval
of the Administrative Officer. Three (3) copies of the approved revised plans shall be
submitted to the Administrative Officer prior to permit issuance.
a) The landscaping plan shall be revised to be revised to include the phasing
plan and also be revised to divide the planting schedule into the three (3)
phases including the budget for each phase.
4) The applicant shall obtain a zoning permit within six (6) months pursuant to
Section 17.04 of the Land Development Regulations or this approval is null and
void.
5) The Applicant shall obtain a Certificate of Occupancy/Compliance from the
Administrative Officer prior to the use of each new building.
6) Any change to the site plan shall require approval by the South Burlington
Administrative Officer or the Development Review Board.

PLEASE NOTE: Pursuant to 24 VSA §4465, an interested person may appeal this decision
by filing a Notice of Appeal with the secretary of the Development Review Board. This
Notice of Appeal must be accompanied with a $233 filing fee and be filed within 15 days
of the date of this decision.
