City's Land Development Regulations. Five (5)1 regular size copies, one reduced copy (11" x 17"), and
one digital (PDF-format) copy of the site plan must be submitted A site plan application fee shall be
paid to the City at the time of submitting the site plan application in accordance with the city's fee
schedule.
Administrative site plan applications require three (3) regular size copies, one reduced copy (11" x 17"), and one
digital (PDF-format) copy.

NOTE: NOTIFICATION of ADJOINING PROPERTY OWNERS: Notification of adjoining
property owners, in accordance with 24 V.S.A. §4464(a) and Section 17.06(B) of the South Burlington
Land Development Regulations, is the responsibility of the applicant. After deeming an application
complete, the Administrative Officer will provide the applicant with a draft meeting agendas or public
hearing notice and sample certificate of service. The sworn certificate of service shall be returned to the
City prior to the start of any public hearing.

PROJECT NOTES
The landscape needs of 22 Patchen Rd. are a new
walkway connecting from the Patchen Rd sidewalk
to the house addition, landscaping the pathway for
aesthetics, and providing a hedge for screening in
the rear parking lot. The walkway has been
proposed for four feet wide to be constructed of
concrete or pavers. Landscape shrubs meander
along the walkway and an additional foundation
planting is created along the existing house. The
hedge is addressed by a mass planting of
arborvitaes at 5' intervals.

The purpose of this application is to permit the proposed uses following a building expansion at 22
Patchen Road: 822.5 sqft nail salon, 629 sqft medical office and 1,035 sqft office space. Currently, the
space is being used for a medical office and office located on 22 Patchen Road in South Burlington,
Vermont.
Dear Ladies & Gentlemen:
I reviewed the application and technical information submitted for the above referenced project.
Unfortunately, there were items that were not included or were insufficient and I was unable to determine if
this project meets the minimum standards of the Environmental Protection Rules. Please provide the following
information:
1. The application states no prior permits were found. Please clarify what existed uses etc. as of 1/1/07
with detailed basis of design.
2. Pedicure chairs are similar to hair salon chairs therefore the flows are 15o gpd /chair. Additional
allocations will be needed from the municipality for water and wastewater.
3. Part V Section C has 356 gpd increase and 365 total which appears to be a typo. Please revise the
application page and upload it to eDEC.
4. Please revise any necessary pages of the application materials related to the previous comments and'
upload to eDEC.
Please respond within thirty, (3o) days from the date of this letter. Re -submittals that are incomplete or made
after thirty days may result in the denial of the project. I will continue my review of the project once I receive

The City Stormwater Section has reviewed the "Hannah & NGA Tran — Proposed Site Plan" prepared by NLB Construction
Consulting, LLC, dated 5/5/15 and last updated on 8/17/15. We would like to offer the following comments:
1. The proposed project is located in the Potash Brook watershed.
2. It is recommended that dimensions of the pipe and stone are added to the infiltration trench detail, as well as a
more defined swale above the trench.
3. Work in the City Right Of Way (ROW) requires a permit before construction can begin. A "Permit to Open Streets
or Right -Of -Way" can be obtained from the South Burlington Department of Public Works on their web site, or
by stopping by their office located at 104 Landfill Road.
4. The DRB should include a condition requiring the applicant to regularly maintain the stormwater treatment
practice.

Ms. Bechtel said the application had received Zoning Board approval.
Mr. Leonard Duffy said Dr. Celani now practiced out of an office on San
Remo Drive. He wants to keep the residential character of this property,
since that and privacy are very important to his clients. Dr. Celani hopes
to have another psychologist or social worker, plus another person, renting
the upstairs. He said Dr. Celani did not want to make provisions for connecting
the parking lot here with the one to the south, for reasons of privacy. Mr.
Poger saw no need to connect the two. He noted, though, that the parking lot
would have to be enlarged to allow for the required number of cars.
Mr. Jacob moved that the South Burlington Planning Commission approve the
site plan application of David P. Celani for the conversion of a residential
structure to an office building at 22 Patchen Road as depicted on a plan
entitled "Site Plan: Offices of David Celani Ps cholo ist" and prepared b
Leonard 'Duffy and Associates dated 4 25 84, with the fallowing stipulations:

This site Plan Application is for the conversion of a 2-story residential structure
to an office building for a Mental Health Consultation Service. The Zoning Board is
considering an application for a variance of the 20 foot driveway width requirement
to allow the existing 9 foot wide drive. If this is not granted, a revised plan will be
submitted for the June 19 meeting. Assuming the variance is granted, my comments are
as follows:
Circulation and Parking: The 9' foot driveway requires that adequate site distance
be maintained both into the rear of the property and onto Patchen Road. Hedges should
not inhibit distances. Average daily traffic flow is projected at 12 vehicles.
Six parking spaces are required and proposed plan is not adequate for six vehicles
at a 9' x 18' space and a 20' aisle. This must be revised, perhaps by moving parking
further towards the rear property line.
Pedestrian and Vehicular Access: The existing curb cut at the 9' wide drive will
be retained.
Landscaping and Screening: The applicant proposed.$1000 in landscaping, well above
the 3% required. A new 36" picket fence is proposed for the Southern Side, and existing
fence will be retained on the east and west property line.
Other: See Bill's memo regarding storm water run-off and a connecting
drive.

1. The adjacent neighbor expressed his concerns regarding the lot drainage
especially if it is paved. I would recommend that a drop inlet be installed
north-west of building and new property line and lot graded toward this in-
let. This inlet could outlet in the Patchen Road drainage.
2. There should be more clearance between the entance drive and the residence
to the south to allow for creening and snow storage. The drive may be de-
creased to a one way drive.

Dear Ms. Bechtel,
Plans were reviewed by this department on the David Celani
Psycholigist office building at 22 Patchen Road. At this time
I do not see a problem with giving fire protection.
My only concern is to make sure that plans are sent into
the department of labor & industry for review because of a change
of use for the building. If you have any questions please feel
free to call me.
